The Hergest Unit at Ysbyty Gwynedd in Bangor has been inspected by Healthcare Inspectorate Wales (HIW), with the report shedding light on both the unit's strengths and areas requiring improvement. The inspection, conducted in September 2025, covered the Aneurin, Cynan, and Taliesin wards, as well as the Mental Health Suite.
Resilience and Teamwork

Patients and staff highlighted the importance of meaningful activities for recovery, despite the availability of activity rooms and a gym, which were underutilized due to staffing constraints.
Record-Keeping and Documentation: The unit faces challenges with paper-based records, including difficulties with completion, storage, and accessibility. Incomplete patient records and medication charts, including missing legally required forms and inconsistent allergy information, were noted, which could impact patient safety.
Environmental Concerns: The overall environment, while generally clean, requires attention to maintain a safe and therapeutic atmosphere. Issues include limited seating in communal areas, garden maintenance needs, and shared bedrooms without en-suite facilities. Safety signage and repairs to facilities, such as water heaters, were also highlighted as necessary.
Staffing and Resource Management: The unit's reliance on temporary agency staff due to persistent staff shortages was noted. Inspectors recommended reviewing the night-shift staffing model on Cynan Ward, where only one registered nurse was on duty, to ensure the skill mix aligns with patient needs.
